Extra Life is an annual charity event in which people play games live to raise money for children in children's hospitals, which Stephen and Mallory have participated in since 2012 to raise money for their local children's hospital, McLeod Children's Hospital in Florence, SC.

Though they started out by streaming Earthbound in 2012 and 2013 with no donation structure, since 2014 their streams have been structured such that viewers can donate to see a game from a certain system be played, such that the game with the highest donation total will be played next. Each game gets played for 30 to 45 minutes, after which the next game is played.

On Vlog Day 3612, Stephen and Chaz came up with the idea for the roulette which was added to Extra Life 2019 and all Extra Life streams going forward. The roulette is another option for viewers who don't want to donate to a specific game or like chaos. At some point during the Extra Life stream, the roulette will be spun and the all games that have at least $25 donated towards it will be eligible to be put on the roulette. Several of the games that are eligible will then be randomly chosen to be put on the roulette. Finally, the roulette will be spun and the game it lands on will get the amount of money currently donated to the roulette added to its donation total.

As of Extra Life 2023, they have raised $663,973 for the McLeod Children's Hospital. Alongside this, 208 unique games have been donated to and played either during an Extra Life event or Bonus Stream.

All the "Money Raised" Amounts are based on the amount of money raised by the end of the stream and doesn't include donations from after the stream (Stephen often keeps the Extra Life donations open until the end of the year).
